Lakeland Homecare Solutions is operated by Four Flag Health Ventures, a for-profit company under the corporate umbrella of Lakeland Health Systems, with headquarters in St Joseph, located in beautiful southwestern Michigan.

Lakeland Health Systems owns and operates two acute care hospitals in Niles and St Joseph, one rehabilitation hospital in Berrien Center, Michigan, and two Long Term Care facilities.

Lakeland has been in the home health business since 1987, and consists of six separate entities:

♦ Lakeland Hospice

♦ Lakeland Home Medical Equipment

Lakeland Homecare Solutions was formed in 1994 when other hospitals approached Lakeland and asked for help starting their own home care business units. Since 1994 Lakeland Homecare Solutions has helped 11 hospitals and 2 stand alone, for profit companies start home health care agencies. Lakeland Homecare Solutions grew primarily by word of mouth into a major presence in southwestern, central, and eastern Michigan, and northern Indiana.

Presently there are ten Medicare certified home care entities in the consortium, as well as two hospice, two private duty, and one home infusion company. These separate entities come together every couple of months for education and networking purposes.

Lakeland Homecare Solutions also provides ongoing consultation, management support, computer services, billing and financial services. Our goal is to develop a long term relationship with our affiliate agencies and continue to help them be successful.
