Methodist Hospitals Home Health Mission
We believe that each person we care for is an individual
who deserves the very best healthcare delivered with
respect and dignity. Methodist Hospitals Home Care
provides quality, innovative and cost-effective home
care and related health services for our patients and
the physicians who entrust their patient’s home care
needs to our professional staff. We strive to
continuously improve the quality of that care.
Home Health Care
With hospital stays
becoming shorter, home health care is becoming more
common. Methodist Home Care nurses offer a wide range of
services while keeping the patient's own physician
informed. They also provide education and instruction to
family, caregivers and patients on the disease,
treatment plan, medications, injections, IV infusions,
dressing changes, and diet. Methodist Hospitals Home
Care Services are accredited by the Joint Commission on
Accreditation of Healthcare Organizations and is
licensed by the Indiana State Board of Health.
The Methodist Hospitals Home
Health Services Offers:
Skilled Nursing
Nurses focus on patient comfort and provide
education and instruction to family, caregivers, and
patients on the disease, treatment plan, dressing
care, diet, and medications including IV
medications.
Rehabilitation Services
Physical, occupational, nutritional and speech
therapies are available upon recommendations from
the interdisciplinary group.
Personal Care
Serving under a nurse’s direction, Home Health
technicians assist in daily living activities such
as bathing, dressing, and ambulatory assistance.
Spiritual Services
Our Spiritual Care program offers emotional and
spiritual support for patients and loved ones.
Bereavement counseling is available for surviving
members and caregivers after the death of a patient.
Congestive Heart Failure Program
At the Northlake and Southlake campuses, Methodist
has initiated an innovative Congestive Heart Failure
Program. The goals of the program are to reduce
readmission rates to the hospital and to provide the
best care and education to promote health
independence

Patients are visited in the home by an RN who
will assess and instruct on medications, weight
monitoring, diet restrictions, activity, and
disease process.

Physical and occupational therapists are
available as needed.

Home Care services are typically covered under
Medicare, Medicaid and other insurance plans
following hospitalization.

Once a patient is ready for discharge from home
services, an RN will contact them every two
weeks by phone. The patient’s physician will
then be contacted for follow-up instructions.
