Pennock Homecare now brings modern medical technology
into your home. Today hospital stays are shorter and
therapies are highly specialized. Homecare services
often permit earlier hospital discharge with many
therapies being performed at home. This allows for
healing and teaching of therapy in familiar, comfortable
surroundings, enhancing privacy, independence and a
partnership in recovery.
Homecare Services would like to be your partner in
recovery. We offer a smooth transition of care from
physician or hospital to home services. Care begins with
a phone call; soon after that nurses, aides and/or
therapists will visit the patient at home to provide the
needed services. Pennock Homecare will handle all of the
billing, relieving you of paperwork concerns. We accept
assignment from Medicare and Medicaid and participate
with Blue Cross/Blue Shield and many other private
insurances.
Registered Nurse (RN) will open every case with an
assessment and an explanation of services. Nurses may
change surgical and other wound dressing, give
injections, draw blood and assess the patient per
doctor's orders. Nurses also teach the patient, or
others assisting the patient, how to perform self-care,
as well as educate the patient on symptom, pain and
medication management.
Psychiatric Nursing - Under a physician's order, the
psychiatric nurse provides the skilled nurse assessment
along with mental assessment, emotional, behavioral and
psycho social assessments as well as reassessments. The
nurse will enhance mental health wellness and educate
the patient family and staff in meeting the related
needs.
Physical Therapist (PT) will evaluate and treat the
patient in conjunction with the physician's
prescription. Therapy may include exercises to restore
function, instruction on the use of assistive devices,
observation and measurement of progress.
Occupational Therapist (OT) under a physician's order
may assist with activities of daily living, exercises to
increase motion, and evaluate adaptive equipment to
increase independence.
Speech Therapist (ST) also provides therapy in
conjunction with a physician's prescription, which may
include teaching and training designed to assist with
speech and swallowing difficulties.
Medical Social Worker (MSW) provides social casework
to patients and their families. This includes guidance
counseling, and assistance with social, economic and
psychological needs, as well as long term planning.
Home Health Aide (HHA), under the direction and
supervision of an RN, provides personal care in the
absence of a willing and able caregiver.
Pennock Homecare Services does not deny admission to
persons with a contagious disease including, but not
limited to HIV, MRSA and Hepatitis B unless otherwise
directed by the State Health Department. Patients will
be accepted regardless of race, religion, color,
national origin, age, gender, sexual orientation,
height, weight, marital status or disability.
